Our GP practice has joined with Magdalen Medical Practice and Oak Street Medical Practice to form Norwich North PCN.
PCNs bring health care partners together to enable more personalised and coordinated health and social care services for people closer to home.
The aim is to improve care and health for people locally so they can stay safe and well closer to home, and also to make primary care services more resilient.
Patients will continue to remain registered with their own GP practice, but working as part of a PCN allows us to share expertise and resources.
