Use this service to get a sick (fit) note for work following an absence of more than 7 days.
You can use this service if you:
- are registered at the surgery
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of

Please note urgent appointments are only provided for genuine medical emergencies and not to provide sick (fit) notes. It is possible for a fit note to be back dated so you do not need to be seen urgently or on the day that your note expires.
